InnerVista Psychology & Assessment Centre ("InnerVista", "we", "us", or "our") is a psychology and neuropsychological assessment practice led by Dr Samantha McLean, Clinical Neuropsychologist. We are located at The Entrance Medical and Specialist Centre, 253 The Entrance Road, The Entrance, NSW 2261. This policy applies to information collected through our practice and this website.

2. The information we collect

The information we collect depends on your relationship with us. It may include:

  • Personal information such as your name, date of birth, address, telephone number, email address, and emergency contact details.
  • Sensitive and health information such as medical history, presenting concerns, assessment results, clinical notes, diagnoses, treatment and referral details. The law treats health information as a special category requiring additional protection.
  • Information about children where we provide services to a child or adolescent, including information provided by parents, guardians, schools, and other professionals with appropriate consent.
  • Payment and billing information, including Medicare, NDIS, or private health fund details where relevant.

3. How we collect your information

We collect information directly from you wherever possible — for example, through enquiry forms, appointments, consultations, and assessments. With your consent, we may also collect information from third parties such as referring GPs and specialists, schools, other health professionals, parents or guardians, and Medicare or the NDIS.

4. Why we collect and how we use your information

We collect and use your information to:

  • Provide psychological and neuropsychological assessment, diagnosis, and therapy services;
  • Communicate with you about appointments, results, and your care;
  • Prepare reports and correspondence with your consent;
  • Process billing, rebates, and claims;
  • Meet our professional, legal, and record-keeping obligations; and
  • Maintain and improve our website and services.

5. Disclosure of your information

We treat your information as confidential and will only disclose it where you have consented, or where disclosure is required or authorised by law. With your consent, we may share relevant information with referring practitioners, treating GPs and specialists, schools, and other health or allied health providers involved in your care.

There are limited circumstances in which we may be required to disclose information without consent — for example, where there is a serious and imminent risk to the safety of you or another person, where a child is at risk, where disclosure is required by a court or by law, or as otherwise permitted under the Australian Privacy Principles. We do not sell your personal information.

6. Storage and security

We take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification, or disclosure. Records may be held in secure electronic systems and/or physical files with restricted access. Where information is stored or processed by third-party service providers, we take reasonable steps to ensure they maintain appropriate security and privacy standards.

7. How long we keep your information

We retain health records for the periods required by law. In New South Wales, this is generally a minimum of seven years from the date of last contact for an adult. For a person who was under 18 at the time of last contact, records are generally retained until the person reaches 25 years of age. After these periods, records are securely destroyed or de-identified.

8. Accessing and correcting your information

You have the right to request access to the personal and health information we hold about you, and to ask us to correct it if it is inaccurate, out of date, or incomplete. To make a request, please contact us using the details below. We may need to verify your identity before responding, and in limited circumstances permitted by law we may decline access and will explain why.
